What is Supported Living ?

Supported Living 

The supported living model gives individuals power and choice over their own lives, while supporting each client based on their individual needs.

 

Supported Living is a service that provides clients support in their home as well as in their community. Individuals typically live with two or three housemates, and our staff provides support based on their individual needs. Support may vary from a few hours per month, up to 24 hours/day.
Provided support is determined by a Developmental Disabilities Administration (DDA) assessment.

Community Activities, Daily living Activities,
Building relationships  

 

Community Inclusion provides support for people with developmental disabilities to participate in integrated activities, events and volunteer opportunities in ways that are similar to other people their same age. 

 

Community Inclusion services provide individuals with one-one-one support to develop significant relationships in the community through participation, contribution & inclusion with the goal of active and independent engagement in their community.

Finances, Shopping, Self-advocacy

Clients we support have a payee services representative that assist in managing their finances. We take care to understand to the best of our ability how each individual’s life situation intersects with their financial health.

Excel works closely with client payee representatives and support our clients with a wide range of financial services, including building budgets, paying bills, receiving Social Security benefits, managing expenses, and general financial education. Our goal is to help people learn healthy financial habits that allow them to stabilize their lives and build long-term financial independence.

Power of Choice 

We support individual choices by creating the framework for person centered planning, direct quality supports and services, advocate for resources, drive policies and system evolution and provide a way to evaluate our efforts. We recognize that people with intellectual and/or developmental disabilities are
self advocates and contributing citizens who enrich their communities across their lifespans. 

​Power is having the ability to direct or influence our own lives. Information and experiences are basic to making choices. Choice is the opportunity to make decisions that are right for ourselves, and include the opportunity to succeed or fail. Power and choice means to: recognize what we want and how to achieve it, express and pursue our own opportunities take a personal risk, pursue dreams and goals, choose who supports and encourages us in our lives. – Access accurate information, maintain respect, privacy, individuality, cultural identity and honor dignity of risk.
